Why I Always Review My Anti-Wrinkle Patients 2–4 Weeks Later

One of the questions I’m asked most often is:

“Why do I need to come back for a review if my treatment has already been done?”

It’s a fair question, but for me, the answer is simple.

The review appointment isn’t an optional extra – it’s an essential part of achieving the natural, balanced results my patients expect.

Over nearly 30 years as a doctor, including more than 22 years specialising in medical aesthetics, I’ve learnt that anti-wrinkle treatments aren’t just about the injections themselves. It’s about treating the individual, not just administering a product, and assessing the final result before making any further decisions.

For most patients, I recommend a review appointment between two and four weeks after treatment. However, treatment is always personalised. Patients I’ve been treating for many years may not always need to return for a review, as we have a detailed understanding of how their muscles respond over time. That said, the opportunity to review their results is always there if needed.

Is Botox the Same as Anti-Wrinkle Treatment?

Before we go any further, it’s worth clearing up one of the most common misconceptions.

Many people use the word Botox to describe all anti-wrinkle injections. In fact, Botox® is simply one brand of botulinum toxin used to temporarily relax selected facial muscles that contribute to expression lines.

At Dr Preema London Clinic, we generally use the term anti-wrinkle treatment or anti-wrinkle injections, as this more accurately describes the treatment itself rather than a specific brand.

Whatever you choose to call it, the principle is the same. The treatment works by temporarily relaxing carefully selected muscles to soften lines while maintaining natural facial movement. My aim has never been to remove every expression or create a frozen appearance, but to help my patients look refreshed, well-rested and naturally themselves.

One of the reasons I insist on reviewing my patients is because achieving that balance doesn’t end when the injections are finished – it comes from carefully assessing how your individual muscles respond once the treatment has fully settled.

No Two Faces Respond the Same

One of the reasons I love aesthetic medicine is because every patient is unique.

No two faces have the same muscle strength, facial anatomy or patterns of movement. Even the left and right sides of the same face can behave differently.

Before I treat any patient, I assess how their facial muscles move, where they’re strongest and how they’re likely to respond to treatment. That experience guides every decision I make – from how much product to use, to where I place it, and sometimes just as importantly, where I choose not to treat.

Years of experience allow me to predict how a treatment is likely to develop, but no two people respond in exactly the same way. The review appointment allows me to confirm that your muscles have responded as expected and, if needed, make small refinements based on your individual response.

For me, that’s what truly personalised treatment looks like.

Why I Ask You to Wait

One of the biggest mistakes people make is judging their anti-wrinkle treatment too soon.

While some patients begin to notice changes within three to five days, the full result usually develops over 10 to 14 days. Stronger muscles can sometimes continue settling for a little longer.
If I review you after only a few days, I’m assessing a treatment that hasn’t actually finished working.

Waiting until the treatment has fully developed allows me to see exactly how your muscles have responded before deciding whether anything needs adjusting.

A Review Isn’t About Having More Product

Some patients worry that coming back for a review automatically means they’ll need more treatment.

In reality, that’s rarely the case.

Most of my review appointments end with me saying exactly what every patient wants to hear:

“Everything has settled beautifully. I’ll see you in a few months.”

If your muscles have relaxed evenly, your facial movement remains natural and we’ve achieved the result we planned, there’s nothing more to do.

If I do recommend a small adjustment, it’s because I believe it will improve balance—not because it’s routine.

One of the principles that has guided my practice throughout my career is that natural-looking results are achieved through precision, not excess. It’s never about using more product than necessary—it’s about using the right amount, in the right place, for the right patient.

My goal has never been to create frozen foreheads or expressionless faces. I want my patients to look refreshed, rested and confident, while still looking like themselves.

Sometimes the best clinical decision I make is deciding not to treat further.

That’s exactly why the review appointment is so valuable. It allows me to make that decision with confidence, based on the final result rather than assumption or routine.

Why I Don’t Recommend Waiting Longer Than Four Weeks

Patients occasionally ask whether they can come back several weeks, or even a couple of months later, if they feel they need a small adjustment.

The answer is usually no, and there’s an important clinical reason why.

The ideal time to review an anti-wrinkle treatment is when it has reached its full effect, but before the muscles begin to regain movement. This gives me the clearest picture of how your face has responded and whether any adjustment is genuinely needed.

After around four weeks, the effects of treatment gradually begin to wear off. For many patients this happens evenly, but every face is different and some muscles may begin to regain movement before others. At that stage, it becomes much harder to judge whether any difference is simply part of the natural wearing-off process or whether it relates to the original treatment. Adding more product at that point could create an imbalance rather than improve the result.

By reviewing patients between two and four weeks, I’m assessing the treatment once the final result has fully developed. If a small adjustment is needed, I can make that decision with confidence, knowing I’m assessing the true outcome rather than a treatment that’s already beginning to change.

After four weeks, any further treatment should be considered a new treatment rather than a refinement of the original one.

Every Review Makes Your Next Treatment Better

The review appointment isn’t just about today’s result.

It’s about every treatment that follows.

Each time I review a patient, I learn more about how their face responds. I learn how quickly the treatment develops, how long the result lasts and whether certain muscles naturally need more or less support than others.

Over time, I also build a clearer picture of how you respond to different botulinum toxin products. While they all work in a similar way, some patients find they respond better to one product than another or that their results last longer with a particular brand. That’s another reason why continuity of care is so valuable.

All of this information helps me personalise your treatment even further. Rather than repeating exactly the same approach every three or four months, I continually refine your treatment plan based on how your face responds.

That’s how natural-looking results become even more consistent over time.

Why My Patients Return Year After Year

Many of my patients have trusted me with their treatments for years. Some have now been coming to me for more than 20 years.

That continuity of care is something I never take for granted.

When I’ve treated someone over time, I develop a deep understanding of how their face changes, how they respond to treatment and how their goals evolve. I also learn how they respond to different botulinum toxin products, allowing me to tailor treatment even more precisely.

Just as importantly, I know when anti-wrinkle treatment may no longer be the whole answer.

As we age, the changes we see in the face are rarely caused by muscle movement alone. Skin quality, collagen loss, hydration and structural support all play a role. My Philosophy Has Never Changed

Products have evolved.

Techniques have advanced.

The world of aesthetic medicine continues to change.

But my philosophy has remained exactly the same.

Every patient deserves an individual assessment.

Every treatment plan should be tailored to the person sitting in front of me, not simply repeated because it worked last time.
Sometimes that means refining an anti-wrinkle treatment. Sometimes it means introducing another treatment to address changes in skin quality, collagen loss or facial structure. The right approach is always the one that’s right for you at that moment in time.

That’s why the review appointment is so valuable. It’s not simply about checking whether your treatment has worked. It’s about assessing the result, understanding how your face has responded and making informed decisions about what comes next.

Because beautiful, natural-looking results aren’t created by using more product.

They’re created through experience, precision and knowing when to treat, when to wait and when a different approach will achieve a better outcome.

Why do you review anti-wrinkle treatments after 2–4 weeks?

Because that’s when the treatment has fully settled. Reviewing the result too early can lead to unnecessary adjustments before the muscles have finished responding, while waiting too long means the muscles may already be recovering, making it harder to accurately assess the original result.

What happens if I miss my review appointment?

Your treatment will still work, but the review gives me valuable information that helps personalise your future treatments and maintain consistent, natural-looking results over time. If more than four weeks have passed, any additional treatment would usually be considered a new treatment rather than a refinement of the original one.
